Change Confirmation Form (B155)
This Form is to be used each time there is a change in a building contract. It is important to record all changes in case there is disagreement on what change was agreed. This Change Confirmation Form will briefly describe the change as well as the cost and any adjustment to the completion date for the work.
As with all ContractStore documents, it is in MS Word and you can prepare a clean copy for each variation that occurs on your building contracts.
